Brown Emergency Medicine Rental

Come Play at Providence Children's Museum!

This form is to register for Brown Emergency Medicine's PCM facility rental on Sunday, 5/1/22 from 9:30 AM - 12:30 PM.

PCM is at the forefront of safety. All staff and visitors ages 2 and up, regardless of vaccination status, must wear a mask (surgical or N95 preferred) that covers their nose and mouth at any time while in the building. No exceptions can be made to this policy. Forgot yours? We have extras at the front desk.

If anyone in your group is experiencing symptoms related to COVID-19 or has been exposed to someone with COVID-19 within the past 14 days, please visit PCM another day. Find further information about our COVID-19 policies here.
